Cyclopia

Cyclopia is a rare (40:10,000 in embryos) form of holoprosencephaly and is a congenital abnormality (birth defect) characterized by the fusion of the orbits into a single cavity containing one eye. Typically, the nose is either missing or replaced with a functioning nose in the form of a proboscis. It is generally thought to be caused by ingesting Veratrum californicum (Western False Hellebore) or other wild plants containing toxins on the 14th day of gestation.
