As of 2004, the highway's northern terminus is Columbia, Tennessee at U.S. Route 31. Its southern terminus is in Mobile, Alabama at U.S. Route 45.

The highway passes through the following states:
- Tennessee
- Alabama. Between Mobile and Fayette and again from Killen to the Tennessee state line, U.S. 43 is paired with unsigned Alabama State Route 13. Between Fayette and Hamilton, U.S. 43 is co-routed with unsigned Alabama State Route 171. Between Hamilton and Florence, the unsigned partner for U.S. 43 is Alabama State Route 17.
